Thrive in comfort - 24/7 personalized care in a small residential home, with expert support for medication and complex health needs.
At Bethel Oaks Assisted Living, we specialize in high-acuity care and safe, efficient hospital-to-home transitions. As a pharmacist-owned and operated residence, we provide advanced medication management, reducing the risk of errors, complications, and hospital readmissions.
Our boutique 5-bed residence offers both private and shared room options, allowing us to maintain a low caregiver-to-resident ratio and deliver highly personalized, attentive care. This smaller setting ensures each resident receives the time, oversight, and individualized support they need—especially during critical post-discharge periods.
We are equipped to accept residents with complex medical needs, including those requiring medication administration, chronic disease management, and enhanced levels of care in a residential setting. Our experienced team works closely with discharge planners, case managers, social workers, and home health providers to ensure seamless care coordination and continuity of care.
From daily assistance to meals and companionship, every service is tailored to meet individual needs.
We start with a conversation to learn about your loved one’s health, preferences, and care requirements to ensure the right fit.
Visit our home, meet our team, and complete a personalized care assessment to create a plan tailored to your loved one.
We guide you through paperwork, pricing, and next steps—making the transition clear, smooth, and stress-free.
From a warm welcome on move-in day to continuous updates and care adjustments, we support both residents and families every step of the way.

We provide careful oversight of all medical and medication needs, ensuring everything is followed correctly and consistently. As a pharmacist-owned residence, we bring an added layer of expertise to medication management, helping reduce risks and improve overall safety.
Our home is staffed 24/7, and every room is equipped with an emergency call system. Combined with our high staff-to-resident ratio, this ensures that help is always immediate and residents are never left unattended in urgent situations.
We understand that this transition can feel overwhelming. Our goal is to take the burden of daily caregiving off your shoulders—handling medications, meals, and safety—so you can focus on being present as a son, daughter, or spouse again.
Absolutely. We encourage residents to bring personal items and familiar belongings to create a comfortable and meaningful living space that truly feels like home.
We focus on active, engaging living. Residents enjoy home-cooked meals, social interaction, and activities tailored to their interests. We don’t just manage time—we create opportunities for connection, comfort, and joy each day.
Our visiting hours are 7AM – 9PM. We strongly encourage visits, as staying connected with loved ones is an important part of overall well-being.

I have been living here for a few months and I love the friendly staff and residents. We have a lot of activities available and one of my favorite things to do is movie night together.

My mom moved there two years ago and is very happy there. The staff is pleasant, helpful and caring and they treat him like family. The management does a good job of keeping us informed on matters related to the facility and any impact on residents.

I am so glad I could move in after several years of living alone and caring for a large house. I have many friends now to talk to and participate in activities with.

They were the best for us. I wish my loved one had moved there sooner. They have great activities. The care they give to the residents is fantastic. The food was good for our loved one.
